Full Official Terms:  

Effective Date: July 14th, 2025 

1. Eligibility:

• This survey is open to residents of Canada who are eighteen (18) years of age or older at the time of participation. 

• Employees, representatives, agents, and affiliates of MUCHO Burrito including their immediate family members or those living in the same household, are not eligible to participate in this survey. 

2. Survey Participation:

• To participate, guests must complete the Tossed Waffle Fries Feedback Survey (the “Survey”) through the provided link or QR code. 

• The survey must be completed between July 14th, 2025 at 12:00AM and August 3, 2025 at 11:59 PM. 

3. $5.00 Gift Card Offer:

• Upon successful completion of the survey, participants will receive a $5.00 digital gift card for completing the survey. 

• Gift cards will be sent out via email to the email address provided by participants during the survey.
   
• Gift cards will be issued commencing upon July 14th, 2025. Please allow up to ten (10) business days for delivery of the gift card following Aug 3, 2025.

4. General Terms:

• Only one (1) survey submission per person is allowed. Duplicate or incomplete entries will not be eligible to receive the gift card. 

• Participants are responsible for ensuring the accuracy of their contact information. MUCHO Burrito is not responsible for lost or misdirected gift cards due to incorrect or incomplete information provided by participants. 

• Gift cards are non-transferable, non-refundable, and cannot be redeemed for cash. 

5. Feedback Use:

• By participating in the survey, you consent to MTY Franchising Inc. collecting and using your feedback for internal research and product improvement purposes. 

7. Modification and Termination:

• MTY Franchising Inc. reserves the right to modify or terminate this survey and its associated terms at any time without prior notice. 

8. Limitation of Liability:

• MTY Franchising Inc. is not responsible for technical issues, delays, or errors that may arise during survey participation or gift card delivery.
